A surgery performed out of habit, not evidence
During the 1980s and 90s, knee arthroscopy — cleaning or scraping the inside of the joint to treat osteoarthritis pain — became one of the most common orthopedic surgeries in the world. The logic seemed obvious: if damaged tissue inside the knee is causing inflammation, removing it should relieve the pain. It had never been rigorously tested against not operating at all.

A un tercio se les hizo la artroscopia real. A otro tercio se les hizo un lavado simplificado. Y al tercio restante, se les practicó una cirugía completamente falsa: los sedaron, les hicieron las incisiones habituales en la piel, simularon los sonidos y movimientos de una operación real, y los cosieron sin tocar la articulación en ningún momento.

The experiment nobody had dared to run
In 2002, surgeon Bruce Moseley, from Baylor College of Medicine, published a trial in the New England Journal of Medicine involving 180 patients with severe knee pain — some unable to get up from a chair without help. A third received the real arthroscopy. Another third received a simplified lavage. And the remaining third underwent a completely fake surgery: they were sedated, given the usual skin incisions, the sounds and motions of a real operation were simulated, and they were stitched up without the joint ever being touched.

A result that changed orthopedics
All three groups improved almost identically. Neither in pain nor in knee function was there a significant difference between those who received the real surgery and those who only went through the full ritual of an operation — operating room, anesthesia, scar — with no real procedure inside the joint. Patients in the sham-surgery group didn't find out until the study's follow-up ended.

Para 2016, al menos cuatro procedimientos ortopédicos más habían mostrado el mismo patrón al compararse con una cirugía simulada: sin beneficio adicional demostrable frente al placebo quirúrgico. Not an isolated case
Moseley's finding opened the door to repeating the experiment with other surgeries. By 2016, at least four more orthopedic procedures had shown the same pattern when tested against sham surgery: no demonstrable extra benefit over the surgical placebo. The trend has continued since in other fields of medicine, revealing that a good portion of traditional surgery rested more on habit and medical authority than on solid evidence.

What this says about how improvement actually works
This doesn't mean those patients' pain was "imaginary," nor that all surgery is useless — plenty of it demonstrably saves lives. It means a real, measurable part of the improvement we feel comes from believing something is being done for us: the ritual, the attention, the care, the sense that an expert is solving the problem. That component isn't a trick — it's a genuine part of how the body and mind respond to treatment, and science is still learning where one ends and the other begins.
