Neither the patient nor the doctor is doing anything wrong — there's no deception or measurement error involved. What happens is that the nervous system responds to a symbol of authority and vulnerability — being evaluated, possibly diagnosed — with a real, measurable physiological reaction. It's a reminder of something we already saw with the placebo effect and sham surgery: the body doesn't fully distinguish between an actual threat and the context we associate with it. Perception, in this case literally embodied in a number on a medical device, can weigh as much as the physical fact it's supposed to measure.
